I know it was with On/ITV Digital, but apparently (so their tech guys said) it was because they transmitted in three different frequency bands, and most people could only pick up one or two with their existing aerial, not all three. Of course the trick to it all was to predict when ITV Digital was going to go bust, then purchase a 12 month subscription package 11 and a half months earlier. Then, when your subscription runs out and they want their set-top box back, there is no-one working at the company available to write to you asking for it back.
