Sounds similar to the medical care/end of life choices portion of the family trust that my former spouse and I established and filed when we were married. We each designated people who would make decisions about our health care (when to turn off the machines, when to not resuscitate) should one or the other of us also be incapacitated. It's a pretty routine thing to do, especially for someone who has been in a hospital for nearly five weeks.
